Read through the sections below, make your selections, and submit. Enrollment locks in your Grandfathered Rate as of today’s date. If you have more than three pets, note it in the form and we’ll follow up to add them.

SECTION 1: GRANDFATHERED RATE ACKNOWLEDGMENT

1.1 Rate Lock
By enrolling in a recurring schedule, the client’s service rate per pet is locked at the prices
confirmed in Section 3 as of the enrollment date (“Grandfathered Rate”). SavyTails may adjust
pricing for new or non-recurring clients at any time with 30 days’ written notice. The Grandfathered
Rate applies exclusively to appointments made under this active recurring schedule and is personal
to the enrolling client.
1.2 Rate Lock Conditions
The Grandfathered Rate applies only while the client maintains active Recurring Client status as
defined in SavyTails’ Client Terms of Service. Please initial each item below to confirm
understanding.
I understand that Recurring Client status and the Grandfathered Rate are forfeited upon
three (3) strikes
— defined as any combination of cancellations within 24 hours, no-shows, or
no-access events totaling three (3). (See SavyTails Client Terms of Service §1.3 for full policy.)

the Grandfathered Rate will not be reinstated upon re-enrollment unless expressly agreed in
writing by SavyTails. Initials:

I understand that voluntary removal from the recurring schedule forfeits Recurring Client
status and the Grandfathered Rate.

SECTION 2 — DEPOSIT & PAYMENT ACKNOWLEDGMENT

I understand that as a Recurring Client, I was charged a $25.00 deposit for my first
appointment only. No deposit is required for subsequent appointments under this active
recurring schedule.


I understand that if my Recurring Client status is forfeited, the standard $25.00 deposit policy
applies to all future bookings.


I authorize SavyTails to charge my card on file for all recurring appointments, including any
applicable same-day cancellation, no-show, or no-access charges, consistent with SavyTails’
Client Terms of Service.

Section 3

PRE-BOOKING AUTHORIZATION & SCHEDULE POLICY

By submitting the form below, the client authorizes SavyTails to:

  • Pre-book appointments on the selected recurring schedule (2-week or 4-week) for each enrolled pet.
  • Send appointment reminders and confirmations via the client’s preferred contact method on file.
  • Charge the card on file for services rendered on each scheduled appointment.
  • Adjust the pre-booked schedule with reasonable advance notice in the event of groomer availability changes, property access changes, or other operational circumstances beyond SavyTails’ control.

3.1 One-Time Schedule Adjustment

The client may request a one-time schedule adjustment by contacting info@savytails.com at least 48 hours before the scheduled appointment. A single one-time adjustment does not count as a strike. Adjustments requested with less than 48 hours’ notice are subject to the standard cancellation policy under SavyTails’ Client Terms of Service.

3.2 Skipped Appointments and Coat Condition

Skipping or significantly delaying a scheduled grooming appointment may result in matting, tangles, or coat deterioration between visits. If the pet’s coat condition at the next appointment requires
additional dematting, deshedding, or corrective grooming work as a result of a missed or delayed appointment, an additional charge will apply. The client will be informed of any additional charges
before they are applied. This policy is consistent with SavyTails’ Client Terms of Service §3.3.

Regular grooming on a consistent schedule is the best way to maintain your dog’s coat health and avoid additional charges. SavyTails recommends staying on your selected 2-week or 4-week schedule for optimal results.

SECTION 4: TRANSFER, SUSPENSION, AND TERMINATION

4.1 Property Agreement Dependency


This Recurring Client Enrollment is associated with the SavyTails service agreement at the property
identified in Section 1. In the event that SavyTails’ service agreement with that property ends for
any reason, this Enrollment is suspended — not immediately terminated — effective on the date the
property agreement ends. The client will be notified of any such suspension as promptly as
practicable.


4.2 30-Day Transfer Window


Upon suspension, the client has thirty (30) days to transfer this Enrollment to a qualifying SavyTails
partner property. If the client transfers to a qualifying property within 30 days, their Recurring Client
status and Grandfathered Rate are fully preserved. If no transfer is made within 30 days of
suspension, this Enrollment lapses and standard terms apply to any future bookings.
To initiate a transfer, contact SavyTails at info@savytails.com within 30 days of receiving suspension
notice. SavyTails will confirm whether a partner property is available at your new location.


4.3 Client Relocation


If the client relocates to a property that is not a current SavyTails partner, the 30-day transfer
window applies from the date of relocation. The client may request that SavyTails evaluate whether
a partnership exists or is being established at their new location. SavyTails makes no guarantee of
service availability at any specific property or location.

4.4 Voluntary Termination


The client may cancel their recurring enrollment at any time by notifying SavyTails in writing at
info@savytails.com. Cancellation takes effect for any appointment scheduled after the written notice
is received. Any appointments already booked within the cancellation notice period remain subject
to the standard cancellation policy under SavyTails’ Client Terms of Service.

Section 5: Client Acknowledgment

By Submitting the form below, I confirm that:

  • My Pet Care Intake Waiver is on file and all information is current
  • I will select my recurring schedule, frequency, and grooming service as described below
  • I have read and understand all three items in the Grandfathered Rate acknowledgment in
    Section 4.2
  • I have read and understood the deposit and payment policies in Section 5
  • I authorize SavyTails to pre-book my appointments as described in Section 6
  • I understand the skip/matting additional charge policy in Section 6.2
  • I have read and understood the transfer, suspension, and termination terms in Section 7
  • I have read and agreed to SavyTails’ Client Terms of Service
